Indications/Uses
Prevents blood clot formation in conditions of the heart, lungs & blood vessels.
Dosage/Direction for Use
Initially 10 mg once daily for the 1st 2 days. Long-term dose: 3-9 mg daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Previous infection which affected the heart. Any condition which may lead to bleeding eg, stomach ulcer, haemophilia, or HTN. Recent surgery/operation. Renal or hepatic impairment. Pregnancy & lactation.
Special Precautions
Risk of calciphylaxis in patients w/ ESRD on dialysis, or in patients w/ known risk factors eg, protein C or S deficiency, hyperphosphataemia, hypercalcaemia or hypoalbuminaemia. Do not take herbal remedy St. John's wort at the same time as warfarin. Sugar (eg, lactose monohydrate) intolerance. Elderly. Childn.
Adverse Reactions
Unexplained bleeding or bruising; feeling & being sick, severe stomach or back pain, diarrhoea, painful blue-purple-coloured toes, mouth ulcers or hair loss; itching, itchy rash or red swollen skin patches; fever or chills, sore throat, ulcers in the mouth or throat, unusual tiredness or weakness; jaundice; dark or cloudy urine, which may have blood in it; swollen face, feet or lower legs.
Drug Interactions
Increased effect w/ anti-arrhythmics eg, amiodarone & propafenone; anabolic steroids; alcohol; NSAIDs especially phenylbutazone & azapropazone; antidepressants eg, amitriptyline; antibacterials especially erythromycin, metronidazole, sulphonamides; antifungals eg, ketoconazole & miconazole; thyroxine, clofibrate, allopurinol, disulfiram, cimetidine, danazol, flutamide, tamoxifen, simvastatin, omeprazole, sulphinpyrazone; cranberry juice or cranberry products (eg, cap or conc). Reduced effect w/ barbiturates, glutethimide, aminoglutethimide, primidone, carbamazepine, OCs, rifampicin, griseofulvin, vit K & sucralfate. Increased or decreased effect w/ phenytoin, corticosteroids, ACTH, cholestyramine. Increased risk of bleeding w/ diflunisal, salicylates, dipyridamole, phenylbutazone.
